I have witnessed first hand similar incidents with owners using these leashes. My vet's office now has a sign up stating that if you do use an extendable leash to make sure it is shortened and in the locked position.
My mother-in-law uses one but she has a papillion that could be walked on a piece of dental floss it is so tiny. What really baffles me is when I see a large lab or pit bull on these types of leashes.

We didn't know any better when we first got Roxy and had her on a retractable leash. It was fine for her because she was easy on leash at the time. But one day we left her with my boyfriend’s mother and she was using the retractable leash to tie her up outside while she was gardening (??). I know the intentions were to keep Roxy with her while she was outside, let her enjoy the fresh air while she was out there, but what?! Use a ROPE! Anyhow, she was transferring the tied bit to another area of the fence when she moved to another part of the garden and this exact situation happened and I have described it the same way when I was blogging about her progress. She bolted and the retractable leash was literally "chasing" her. She ran off into the woods, trying to escape from what was following her.... but it was attached, so she just kept running We didn't find her until the next day, when the stupid retractable leash got caught up in some brush (thankfully) but she had crossed a 4-lane highway and was clearly petrified. We had hung signs and posters some really trashy-like family found her and they only called our posters because we said "reward offered" for someone who finds her. It was literally the worst 24 hours of my life and I will never use a retractable leash again. AND..... Not to mention how many times we have been charged and attacked by dogs on retractable leashes. I’ve seen 25lb dogs pull their owners onto the ground or drag them across the street because the dog “winds up” and gets to the end of the long leash and the unsuspecting owner just goes along for the ride…
